Anonymous Complaint Form
Our office is responsible for investigating allegations of fraud, waste, abuse, and financial mismanagement of university resources. ÌýWe accept direct anonymous reporting of potential irregularities through this internal complaint form and encourage reporting of incidents to the University Police through its Ìýprogram.
In order for your complaint to be evaluated for Whistle-blower determination, you must meet certain statutory criteria under Florida Statute 112.3187, which requires the complainant to disclose their identity and relationship to ¶¶MÅ®ÆÍ.
Recognizing that our resources are limited, and investigations can be time-consuming and sometimes pose legal challenges, we request that any report of suspected irregularities be genuine and unbiased. ÌýIf we choose to follow up on your complaint, coordination will be made with appropriate personnel including legal counsel in gathering and analyzing documents and data for review.
